作用・特徴

ボルチオキセチン（トリンテリックス）はデンマークのルンドベック社がエスシタロプラム（レクサプロ）の次に開発した、セロトニン再取り込み阻害作用・セロトニン受容体調節作用を有する抗うつ剤でS-RIM（Serotonin reuptake inhibitor and serotonin modulator）と称されています。

SSRIと同じセロトニン再取り込み阻害作用に加え、セロトニン5-HT3受容体、5-HT7受容体及び5-HT1D受容体拮抗作用、5-HT1B受容体部分作動作用、5-HT1A受容体作動作用を有し、セロトニンだけでなく、ノルアドレナリン、ドパミン、アセチルコリン、ヒスタミンの遊離を調節します（図1、2）。

このように複数の作用を有することからmultimodal antidepressant：MMAとも呼ばれています。

ボルチオキセチン（トリンテリックス）は悪心、嘔気以外の副作用が低く効果と忍容性が優れていることが報告されています1）。

また、他の抗うつ薬との比較で、うつ病の認知機能改善に有効であることが示されています2）、（図3）。

ボルチオキセチン（トリンテリックス）の認知機能改善においてはセロトニン5-HT受容体を介した作用とともに、他の抗うつ剤よりも高いレベルのBDNF（brain-derived neurotrophic factor：脳由来神経栄養因子）の関与が示唆されています3）、4）。

また、抗うつ薬の治療で抑うつ症状は改善してきても、「以前のように楽しい気持ちになれない」、「感情が乏しくなった」というような状態をエモーションブランティングと呼び、約半数の患者さんが経験していると報告されています5）。

ボルチオキセチン（トリンテリックス）はSSRI、SNRIによる治療中のエモーションブランティングを改善する効果が報告されています6）、（図4）。

一方でSSRI、SNRIは1週間から2週間で効果を認めますが7）、8）、ボルチオキセチン（トリンテリックス）は効果を認めるまで約6週間程かかります9）、（図5）。

また、不安への効果を認めますが10）、パニック障害などの不安障害に対してはSSRIの有効性が現段階では優れています。

効能・効果

日本での保険承認は現在（2022年11月時点）、「うつ病・うつ状態」となっています。

用法・用量

通常、成人にはボルチオキセチンとして10mgを1日1回経口投与する。

なお、患者の状態により1日20mgを超えない範囲で適宜増減するが、増量は1週間以上の間隔をあけて行うこととなっています。

薬物動態

1日1回10㎎を単回で内服した際は血液中の濃度は約12時間で最高濃度に達し、約68時間後に血液中の濃度は半分に下がります（図6）。

毎日内服すると血液中の濃度は約8時間で最高濃度に達し、約65時間後に血液中の濃度は半分に下がります（図7）。食事の影響はありません。

剤型

剤型には10mg錠と20mg錠があります（図8）。

副作用

国際試験及び国内第Ⅲ相試験の集計による副作用発現例は1050例中499例（47.5％）でした。

主な副作用は悪心（19.0％）、傾眠（6.0％）、頭痛（5.7％）、下痢（4.1％）、浮動性めまい（3.3％）でした（図9）。

ボルチオキセチン（トリンテリックス）は副作用の少ない薬ですが悪心・嘔気は生じやすいため注意が必要です。

とはいえ忍容性においては体重増加、性機能障害が少ないという優れた面を有しており11)、12）、有効性においては認知機能改善効果、長期的な感情への安定化作用という優れた面を有しています。
